Do injuries/time wasting/whatever not count in injury time itself then?

They do, but the point Ghost makes is you really do not add on time, you effectively suspend time during the stoppage. So if, for example, the ref adds on 3 minutes, then after 2 of those 3 minutes there is a 5 minute stoppgae for injury, the ref should simply play the remaining one minute when play resumes.

I notice that most commentators don't get this either.

player on the deck for 5 minutes starting 88th +5 = 93rd, then the two remaining minutes of regulation +2 gets us to the 95th.

so there was no time added on.
