The Trident I (C-4) missile fits in the same tube as the Trident II (D-5). The recently went through a life-extension program, probably to replace the solid rocket fuel propellant (which degrades over time). There is no replacement missile.
The differences in the C-4 and D-5 missiles, is the D-5 is significantly bigger and heavier, as such needs beefier gas-generators (to flash more water for launch), different fire control systems. They fly faster and have a longer range and are more accurate, ie pinpoint rather than within 10 meters.
The Ohio class subs have been around for a while, the first USS Ohio rolled out in the late 70's and was commissioned in the early 80s. Like I said they have had thier life extended too, so they will still be around for a while yet. Worth noting, the carry 24 missiles and are the 2nd biggest Submarines ever built (after the Soviet Typhoon SSBN).
