It's usually not given, but if you start OOB and it gives you an advantage, as deemed by the referee, a foul can be given at the spot of the incident.

One of those rarer rules in the book.

My feeling is defenders have learned that distance gives them a bit of a pass and spread themselves beyond what is actually a natural position.

First one wasn't but the second one has to be at least close. As I said he extended his body to close to three times as wide as it otherwise would be.

Well attackers have found about a million ways to act like they got fouled so I'm inclined to believe defenders are finding which millimeter they can stretch to and still be in a natural position.
