They directly cause traffic congestion and indirectly cause accidents.

Should have snipers on the side of the roads really. Spend more than 10 seconds in the middle lane without good reason, boom, you're gone.
 
I have to do a fair bit of motorway driving and middle lane hogging drives me insane, I'll explain why.

Part of my job is managing our fleet, I sometimes use vans etc that are limited in 70mph. The problems I have driving on practically clear motorways having to slow down approaching a slow moving lorry in the inside lane drives me mad. Just because some clown is blocking me, they sit doing about 70 or just under so I sometimes have to slow right down behind a lorry, pull out behind the middle lane div then overtake them in the outside lane.
Now the thing that drives me even more metal is the fact the bell normally sees me overtaking so he speeds up, as the van I'm driving is limited I'm blocked in again with dickheads in BMWs etc flashing me so I have to drop back behind the middle lane div looking like a clown myself.
Honestly, it's a really good thing hand guns aren't accessible over here.

If you ever get the chance to drive in Perth, you will soon stop worrying about the middle lane drivers. Over here they sit in the right hand lane regardless of what speed they are doing or whether they are overtaking. And I'm not talking about 1 or 2. Out of 100 cars 20 will use the left as their default lane, 40 will be in the middle and 40 in the right.

When you come across one i bet you drive right up to their bumper flashing your lights, pull out into the next lane and overtake at 90 mph whilst gesticulating at them don't ya ;)
 
Cyclists riding on the pavement. Motorists parking on the pavement. Pedestrians walking on the road.
People who stop in shop doorways chatting then give you evils for trying to get past.

I honestly did that about 2 years ago and it turned out to be an unmarked police car who proceeded to pull me over. He'd been driving at the exact speed limit on the fast lane of a dual carriageway for over 10 miles, flatly refusing to move over even on occasions where there was nobody in the inside lane for over a mile. I was directly behind him and a huge queue had built up behind me who were getting increasingly aggressive and in the end i lost it and flashed the hell out of him, only for the blue lights to start flashing in the back window, i couldn't believe it
