On Star Trek it says "To boldly go where no-one has gone before" but when they get there why is someone always already there?

1/Earth is in the universe, all other beings are allowed to apply, same as why the World Series winners are always American.

2/They had to send a camera crew first to film the USS Enterprise on landing.
