You can make subs on 4 occasions between kick off and full time : 3 during the game and 1 at half time.

Those 3 can be any time during the game.

And i think you can only make a max of 3 changes at one time, you can't change 5 players all at once.

No, you can only stop the match to make a sub 3 times. You can change 5 players total over those 3 stoppages…. And I think you can also use half-time on top of that.
