So then, what's the point of a proctologist, a doctor that specialises in hoops?

Does the GP only refer you to one if they find an issue that's above their pay grade?

Pretty much, they'd deal with cancer and all those other nasties like crohns etc

Does it help at all if the GP performing the examination is a fit bird?

Really really no, (especially not a new Dr that's about 27/28) you can ask for another Dr to perform the procedure.
But there is a trade off, women tend to have smaller hands. So digital exploration has facets to consider.
