Apologies, let me explain:

In the 343 you posted, I would move Mykolenko into the back three with O'Brien and Tarkowski, and drop Keane. I would then use McNeill to fill the gap on the left of midfield vacated by Mykolenko.

However:

Personally I would prefer to see a flat back 4 composed of a youngster at RB, Tarkowski and O'Brien at CB and Mykolenko at LB. If we genuinely HAVE to play 3 CBs, Keane has provided ample evidence over the last four years that he is utterly incapable of doing so.
