That was one of the two questions I wanted answering: did he kill that lass and her kid? Was his footprint bloody in Arkham from the injuries or did he kill the therapist?
I wondered this but as she didnt do anything bad to him, he might of not, just like the small guy he let go

If you remember the first interview with the councillor, who asks "don't forget the reason you're here in the first place", there is a flashback or flash forward, to Arthur banging his head to what seems to be the same room as the final scene.

Now take the final scene after the fade to black, Arthur is chuckling to himself and another councillor, despite apparently cut due to funding, is back in the same room and asks what's so funny.

Arthur replys, You won't get it.

This to me indicated that it's all in his imagination.

The thought of that had crossed my mind but I thought it was maybe a little too deep and doesn’t really set Joker up as a persona properly. Could be the case though. Find both ways would work well.
