I thought Cleverley moved to left mid, Mirallas shifted to ACM and Barkley dropped to CM after Besic went down, with Pienaar being a like-for-like swap with Mirallas after the second injury.

Nobody played ACM. Lots of people drifted into that space from their normal positions, though. If you want to see what our actual formation is at any given time, look at what how we're set up on goal kicks or when we're established defensively after the other team has had the ball for a little while. When we have the ball, everyone starts drifting around and formations go out the window.
